This webpage provides APHealth SMS terms, privacy information, and help instructions for text-message communications. It is intended to explain how APHealth uses SMS for service-related communication and does not create a guarantee of services, eligibility, benefits, referrals, authorizations, or outcomes.
These SMS Terms, Privacy Statement, and Help provisions apply to service-related text-message communications sent by the April Parker Foundation through its APHealth program.
In this document, “April Parker Foundation,” “APHealth,” “we,” “us,” and “our” refer to the April Parker Foundation and its authorized APHealth employees and representatives.

1. APHealth SMS Terms and Conditions
About the APHealth SMS Service
The APHealth SMS service allows authorized April Parker Foundation employees to communicate directly with APHealth members and program participants through one-to-one text-message conversations.
Messages may concern:
• Appointment scheduling, confirmation, or rescheduling
• Care-management and care-coordination activities
• Follow-up regarding services or previously discussed needs
• Resource and referral information
• Requests for non-sensitive documents or information
• Program participation and service coordination
• Responses to questions initiated by a member
• Other communications directly related to APHealth services
The APHealth SMS service is intended for individual, conversational communication. It is not intended for bulk messages, mass marketing, advertising, or automated promotional campaigns.
Consent to Receive Text Messages
You will receive APHealth text messages only after you voluntarily agree to receive them or initiate a text-message conversation with an authorized APHealth employee.
Consent may be obtained in writing, electronically, verbally during a telephone or in-person conversation, or through another documented opt-in process.
By opting in, you authorize April Parker Foundation and its APHealth program to send service-related text messages to the mobile telephone number you provided.
Your consent to receive text messages:
• Is voluntary
• Is not a condition of receiving APHealth services
• May be withdrawn at any time
• Applies to one-to-one APHealth service communications
• Does not authorize unrelated advertising or promotional messages
APHealth may maintain a record of your consent, including the date, telephone number, method of consent, and employee who obtained the consent.
Message Frequency
Message frequency varies.
The number of messages you receive will depend on your needs, your APHealth services, the conversations you initiate, and whether follow-up is required. Some members may receive only occasional messages, while members actively coordinating services may receive messages more frequently.
Message and Data Charges
April Parker Foundation does not charge you for participating in the APHealth SMS service.
Message and data rates may apply based on the terms of your mobile telephone plan. Your wireless carrier is responsible for any charges related to text or data usage.
Wireless carriers are not responsible for delayed or undelivered messages.
Text Message Delivery
Text-message delivery is not guaranteed. Messages may be delayed, blocked, misdirected, or undelivered because of circumstances outside APHealth’s control, including:
• Wireless carrier or network issues
• Device settings
• Incorrect or reassigned telephone numbers
• Service interruptions
• Full message storage
• Spam filtering
• Loss of cellular or internet connectivity
APHealth is not responsible for losses caused solely by a wireless carrier’s or communications provider’s failure to deliver a message.
Protecting Sensitive Information
Standard SMS text messaging may not be encrypted or completely secure.
If a text message includes health information, APHealth will handle that information according to applicable privacy laws, contractual obligations, and APHealth policies. APHealth may limit SMS content or move the conversation to a more appropriate communication method when needed to protect privacy.
Please do not send highly sensitive information by text message unless an authorized APHealth employee specifically instructs you to use an approved secure method. Highly sensitive information includes:
• Social Security numbers
• Financial-account or payment-card information
• Account passwords
• Government identification numbers
• Photographs of identification documents
• Detailed medical records
• Detailed mental-health information
• Detailed substance-use treatment information
• Detailed diagnostic or treatment information
• Other information you would not want visible on a mobile device or message notification
APHealth employees may limit the information included in a text message and may ask to continue a sensitive conversation by telephone, secure electronic communication, video meeting, in-person meeting, or another approved method.
By choosing to communicate through SMS, you acknowledge that text messages may be visible to anyone who has access to your mobile device, telephone account, lock-screen notifications, or shared telephone number.
You are responsible for protecting access to your device and notifying APHealth when your telephone number changes or is no longer under your control.
No Emergency Services
The APHealth SMS service is not an emergency service and is not monitored continuously.
Do not use text messaging to report a medical, mental-health, safety, housing, or other emergency. Call 911 or go to the nearest emergency department when immediate assistance is needed.
Text messages should not be used when a delay in response could create a risk to your health, safety, housing, or well-being.
Response Times
APHealth employees generally respond during their normal working hours. Response times may vary based on employee availability, weekends, holidays, staff schedules, and the nature of the request.
A text message does not guarantee an immediate response, appointment, referral, authorization, benefit, service, or outcome.
Opting Out
You may stop receiving text messages from an APHealth employee’s telephone number at any time by replying:
STOP
The following recognized opt-out words may also be used:
QUIT, END, REVOKE, OPT OUT, CANCEL, or UNSUBSCRIBE
After you opt out, you may receive one final message confirming that you will receive no further messages from that telephone number.
Because APHealth SMS messages are sent through individual conversations between members and employees, your opt-out may apply to the specific employee telephone number used in that conversation.
APHealth will not intentionally continue sending messages from another number to avoid your opt-out request.
To resume messages after opting out, reply START or provide APHealth with new consent.
Opting out of SMS does not end your eligibility for APHealth services. APHealth may communicate with you by telephone, email, mail, in person, or another agreed-upon method.

Changing Your Telephone Number
Please notify APHealth promptly when:
• Your telephone number changes
• Your telephone is lost or stolen
• Your number is disconnected
• Another person begins using your number
• You no longer control the device receiving APHealth messages
Consent to text one telephone number does not automatically transfer to a new or reassigned number.
Changes to These Terms
April Parker Foundation may update these SMS Terms when its services, technologies, operational practices, or legal obligations change.
The revised version will be posted on this webpage with a new “Last updated” date. Continued use of the APHealth SMS service after an update constitutes acceptance of the revised terms to the extent permitted by law.
2. APHealth SMS Privacy Statement
Scope of This Privacy Statement
This Privacy Statement applies only to information collected, used, or maintained through the APHealth SMS service.
It does not replace any broader April Parker Foundation privacy policy, health-information notice, Notice of Privacy Practices, program consent, managed-care-plan notice, or other privacy document that may separately apply to APHealth services.
Information We May Collect
Through the APHealth SMS service, we may collect and maintain:
• Your name
• Your mobile telephone number
• Your SMS consent or opt-in record
• The date and method of your consent
• Copies or records of messages sent and received
• Dates and times of communications
• Delivery, opt-out, and support-request information
• The identity of the APHealth employee involved in the conversation
• Information you voluntarily provide in a text message
• Information necessary to coordinate or document APHealth services
Text-message communications may become part of your APHealth program or care-management record when appropriate.
How We Use SMS Information
April Parker Foundation may use SMS information to:
• Send and receive APHealth service communications
• Schedule and coordinate appointments
• Provide care-management or care-coordination support
• Follow up on referrals, resources, services, or member needs
• Respond to questions
• Confirm or update contact information
• Document member communications
• Maintain evidence of consent and opt-out requests
• Operate, secure, monitor, and improve the SMS service
• Meet contractual, legal, regulatory, quality-assurance, or recordkeeping obligations
• Protect the safety, rights, property, and operations of members, employees, and the Foundation
APHealth will limit the use of SMS information to legitimate program, operational, legal, compliance, and service-related purposes.
Mobile Opt-In Information
Mobile telephone numbers, text-message opt-in information, and SMS consent records will not be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes.
Text-message originator opt-in data and consent will not be shared with third parties for their own marketing, advertising, or promotional use.
April Parker Foundation does not use APHealth SMS consent to enroll members in unrelated marketing programs.
Service Providers and Message Delivery
April Parker Foundation may use communications platforms, wireless carriers, technology vendors, hosting providers, security providers, and other service providers that assist with transmitting, receiving, maintaining, securing, or supporting SMS messages and related records.
These providers may process information only as reasonably necessary to provide or support the SMS service and are not authorized by April Parker Foundation to use APHealth mobile opt-in information for their own marketing or promotional purposes.
Required or Permitted Disclosures
April Parker Foundation may disclose information when reasonably necessary to:
• Comply with applicable law, regulation, subpoena, court order, or governmental request
• Meet contractual obligations related to APHealth services
• Protect the safety or rights of a member or another person
• Investigate fraud, misuse, security incidents, or unlawful activity
• Protect April Parker Foundation’s rights, systems, personnel, or property
• Respond to an emergency when disclosure is permitted or required
• Work with authorized service providers that support APHealth operations
Where health information is involved, April Parker Foundation will handle the information in accordance with applicable privacy laws, contractual requirements, and organizational policies.
Security
April Parker Foundation uses reasonable administrative, technical, and physical safeguards designed to protect information under its control.
However, no text-message, mobile, wireless, or electronic communication system can be guaranteed to be completely private or secure.
SMS messages may be:
• Stored on your device
• Included in device backups
• Displayed on a lock screen
• Accessed by people who share or obtain access to your device
• Viewed through a shared mobile account
• Retained by wireless carriers or communications providers
• Sent to an incorrect or reassigned telephone number
• Intercepted or disclosed through circumstances outside APHealth’s control
For these reasons, APHealth discourages sending highly sensitive information through SMS.
Record Retention
APHealth may retain SMS records for the period reasonably necessary to:
• Provide and document services
• Maintain program and member records
• Resolve questions or disputes
• Demonstrate consent or compliance
• Complete audits or quality reviews
• Satisfy contractual, funding, insurance, legal, or regulatory requirements
• Enforce April Parker Foundation policies
Records will be disposed of in accordance with applicable April Parker Foundation record-retention and information-security procedures.
Your Choices
You may:
• Decline SMS communication
• Revoke SMS consent by replying STOP
• Request another reasonable communication method
• Ask APHealth to update or correct your telephone number
• Ask questions about how your SMS information is handled
• Notify APHealth that a number should no longer be used
Declining or discontinuing SMS does not prevent you from receiving APHealth services through another available communication method.
