Community Supportive Services

Through our APCares Program we provide beneficiaries in Orange and Los Angeles Counties equitable access to culturally relevant, highly supportive services and valuable resources. Whether it’s healthcare, emergency financial relief, housing, or education, these offerings help promote stability, security, and wellbeing in the lives of historically disinvested communities.

Outreach and Engagement

In all our programs, we undertake direct and partner outreach that is sensitive to the specific needs, and communication styles of the BIPOC populations we serve. We undertake community outreach in the form of community events, canvassing, partnership collaborations, and social media outreach to target prospective beneficiaries. We then identify each beneficiary’s need and work to provide them with the services they need. Outreach methods to engage with eligible applicants are always translated into target languages and disseminated to varied ethnic groups through partners and community-based cultural support organizations.

Rapid Rehousing

Within the framework of our Homeless Prevention services, we work with households who are experiencing a housing crisis or housing insecurity and prevent them from moving into an emergency shelter or living in a public or private place not meant for human habitation. The ultimate goal is to prevent or end homelessness by providing individuals and families with the necessary resources and support to help them achieve and maintain stable housing and economic self-sufficiency.

Homeless Prevention

Emergency Financial Relief

As part of our Homeless Prevention services, we provide households who are experiencing a housing crisis or housing insecurity funding support for housing/rental application fees, rent assistance, advance payments of last month’s rent, utility deposits, and payments, moving costs, transportation expenses, and security deposits as needed.

Social Case Management

Social Case Management includes identifying households of greatest risk and determining the types of support each household needs, and having our Case Managers work with them individually to develop a customized Case Plan for their specific circumstances. Each Case Plan includes a package of multiple services to meet the specific client’s complex needs.
