Case Management

We offer personalized, wraparound support tailored to meet the unique needs of each individual or family we serve. Our experienced case managers begin by building trust and understanding through compassionate, one-on-one engagement. We assess barriers to housing stability, such as financial hardship, mental or physical health issues, employment gaps, or legal concerns, and work collaboratively with clients to develop action plans that promote long-term success.

Through regular check-ins, goal tracking, and referrals to essential resources — including medical care, education, childcare, and job placement — our case managers ensure no one has to navigate the journey to stability alone. Our approach is trauma-informed, culturally competent, and grounded in respect for every person’s dignity and potential.

Housing Navigation

Our Housing Navigation program empowers individuals and families to overcome one of the most daunting challenges: finding and securing a safe place to live. Navigators assist clients with searching for affordable housing opportunities, understanding lease agreements, preparing housing documents, and submitting applications.

We offer support with credit repair, rental history coaching, and housing readiness preparation — giving clients the best chance at securing long-term housing. For those who may face discrimination, language barriers, or systemic challenges, our team advocates fiercely to ensure equitable access to housing opportunities. We also maintain strong relationships with landlords and property managers, facilitating placements and preventing unnecessary delays.

Emergency Housing

When individuals or families experience a housing crisis — such as eviction, domestic violence, or sudden displacement — APHomes offers immediate solutions through our Emergency Housing program.

We provide short-term, safe, and confidential shelter that meets the basic needs of safety, privacy, and dignity. Clients entering emergency housing receive urgent support including hygiene kits, clothing, meals, and transportation assistance. Our crisis response team offers compassionate care while also helping clients plan their next steps, access benefits, and transition into more stable environments.

Emergency housing is often the first touchpoint in a client’s journey with APHomes, and we strive to ensure it is a caring and empowering one.

Interim Housing

Interim Housing serves as a vital bridge between crisis and permanent solutions. This program offers short-term housing for individuals who are no longer in immediate crisis but need additional time, resources, and support before transitioning to independent living.

Residents in our interim housing receive a safe and welcoming environment, while also participating in structured programs such as life skills workshops, employment training, budgeting classes, and wellness activities. Regular check-ins with case managers help clients stay focused on their goals, address ongoing challenges, and prepare for housing readiness.

Our goal is to ensure that by the time individuals exit interim housing, they are equipped with the tools and confidence needed to maintain stability in permanent housing.

Permanent Supportive Housing

Our Permanent Supportive Housing (PSH) program provides long-term housing with built-in support services for individuals and families who face multiple barriers to stability, including chronic homelessness, physical or mental health challenges, and limited income.

At APHomes, we believe in the Housing First model — which means we provide housing without requiring sobriety, employment, or other conditions first. Once housed, clients receive tailored support including ongoing case management, access to healthcare and behavioral health services, peer support, and skill-building opportunities.

We promote community-building within our housing communities, encouraging participation in social events, leadership groups, and neighborhood engagement. PSH isn’t just about a roof over your head — it’s about rebuilding lives and restoring dignity through consistent, compassionate, and holistic support.
